Survey Prize Draw Terms & Conditions - £5 Costa voucher

• The offer is for one £5 Costa coffee voucher per member of staff at the winning branch, up to 20 vouchers maximum. The winner will be randomly drawn on 18 October 2023.
• The promotion is limited to up to twenty £5 vouchers for the winning branch.
• The supplied £5 Costa coffee vouchers are valid until 13/09/2024.
• To qualify for the offer, you must complete the survey sent out by AFI for registered Pawtal users and enter your registration for entry to the draw at the end of this.
• The winning entrant will be notified by a phone call up to 15 working days after the closing date of the survey. The winning branch will then receive their reward fulfilment email from Animal Friends. This email will contain unique codes, the expiry date of these codes, and a link to the redemption page where you can find Costa cafes.
• If the reward email containing your promo code is lost/misplaced or caught in spam filters, Animal Friends will not replace the reward email.
